All applicants are required to provided the following support documentation to their application. Please make sure your documents are correct and up to date. Documents should be attached and emailed along with completed application. Applications will be considered incomplete until documentation is received.
- Proof of Non-Profit Status
- Board Minutes Authorizing the Organization to File a Grant Application
- Roster of Board of Directors and Current Officers
- Organization’s Charter or By-Laws
- Whistle-blower Policy
- If applicable: Cert. of Insurance Naming City of Westerville as Add'l Insured (as relates to funding for events)
- Completed W-9
- Additional pages describing activities/services provided (if needed)
COMMUNITY PARTNERS PLEDGE
Each organization will be required to approve and sign-ff on the following statement as indicated in the application.
As a nonprofit and/or charitable organization applying for and previously funded by public dollars granted by the City of Westerville, I hereby pledge my support for the goal and principles of anti-discrimination and anti-intimidation legislation passed and adopted by Westerville City Council in 2019.
Each applicant pledges to uphold the following:
• Discrimination and intimidation have no place in the City of Westerville, our schools and our society as a whole.
• Organization will foster a community of safety, openness, acceptance, and understanding.
• Organization will neither endorse nor support people, programs and ideas that advance hatred, racism, bigotry, discrimination, intimidation and other divisive ideas and actions.
• Further, Organization will provide no service to people who publicly demonstrate hatred and/or threats of violence, as well as oppression, aggression, micro-aggressions, and discrimination. Specifically, ORGANIZATION deems it unacceptable to take negative actions on a person based on race, ethnicity, gender, religion, sexuality, ability or any other factor.
• To protect our community, Organization will nurture and protect the humanity and dignity of every person in Westerville and served by Westerville.
• Organization understands public funding could be rescinded in the event this pledge is not upheld.
