You will be asked to provide the following information:

  • Please describe why your nominee deserves the award. Please include your nominee’s volunteer organization(s), type of volunteer work and length of service. (Max 250 words)
  • How has the nominee demonstrated exemplary leadership, creativity, cooperation and hard work in their service to others? (Under 250 words)
  • How has the nominee demonstrated exceptional commitment to volunteerism? (Under 250 words)
  • Approximately how many hours has the volunteer provided to your organization?
  • How did the nominee make an outstanding contribution to the community through their volunteer service? (Under 250 words)
  • Optional: Include up to three supporting documents

Nominations will be accepted acknowledging individual and business volunteer activities. Nominees should reside in the City and perform volunteer services within the Westerville community. Qualifying volunteer activities can include formal and informal efforts that benefit people, the environment, animals and more.

Volunteers of the Year will be recognized at the annual State of the Community event.
